Premium Care Plan Membership Agreement

360 Med

This Membership Agreement (“Agreement”) is entered into between the undersigned patient (“Patient” or “Member”) and 360 Med (“Clinic”), and outlines the terms and conditions of the Premium Care Plan Membership provided by the Clinic. By subscribing to the membership, the Patient agrees to the following terms:

1. Membership Overview
The Clinic offers a monthly membership at a rate of £355 / Year. Members are entitled to the following benefits:

a) Annual Health Check
A 30-minute face-to-face GP appointment for a comprehensive health check, scheduled in Month 12 of membership.
If a Member cancels before completing 12 months, they forfeit eligibility for the annual health check and associated services.

b) Comprehensive Blood Test
A comprehensive blood test will be included as part of the annual health check to assess the Member’s overall health status.

c) Follow-Up Based on Results
If a follow-up consultation is required after the annual health check and blood test, it will be booked using one of the Member’s two included GP appointments (see section d below).

d) Additional 20-Minute GP Appointments
Members are entitled to two (2) 20-minute GP appointments per year, which may be used for:

Any general health concerns

Follow-up discussions after health checks

Either face-to-face or remote consultations

e) Subsequent Annual Health Checks
After the first year, further annual health checks (including blood tests) will occur every 12 months, subject to availability.

f) 10% Discount on Additional Appointments
Members and their children (defined as individuals under the age of 18) will receive a 10% discount on any additional GP consultations throughout the membership period.

2. Membership Fees

Membership fees are fixed during the initial 12-month term. Any future fee adjustments will be communicated in writing prior to renewal.

3. Term and Termination
Minimum Term: Membership requires a 12-month commitment, starting from the date of sign-up.

Cancellation: Members may cancel after the first 12 months by providing a minimum of 30 days’ written notice. The membership will remain active until the end of the current billing cycle.

No refunds will be issued for any unused portion of the membership period.

4. Appointment Scheduling
a) Health Check & Blood Test
Members must schedule their health check and blood test in Month 12. Appointments must be booked in advance and are subject to availability.

b) GP Appointments

The 10% discount for GP appointments (for Members and their children) applies immediately from the membership start date.
The two included 20-minute GP appointments can be used at any time during the membership year, including for follow-ups. Appointments are subject to availability.

5. General Terms
Eligibility: Membership is available to individual patients aged 18 or over. Children under 18 are eligible for discounted appointments but are not members themselves.

Non-Transferability: Membership benefits are non-transferable and may only be used by the named Member and their children (under 18).
Non-Refundable: Membership fees are non-refundable.

6. Confidentiality and Privacy
The Clinic is committed to maintaining patient confidentiality. All personal data will be handled in accordance with the Clinic’s Privacy Policy and UK GDPR (General Data Protection Regulation).

7. Liability
The Clinic is not liable for any consequences resulting from missed appointments or failure to follow medical advice. Patients remain responsible for their health and healthcare decisions.

8. Changes to Terms
The Clinic reserves the right to modify this Agreement, including benefits and fees, after the initial 12-month period.

Any changes will be communicated in writing in advance.

If the Member does not accept the updated terms, they may opt not to renew their membership at the end of the current term.

9. Governing Law
This Agreement is governed by and shall be construed in accordance with the laws of the United Kingdom.

10. Acknowledgement
By checking the above box, the Patient confirms they have read, understood, and agreed to the terms of the Premium Care Plan Membership with 360 Med.

Core Wellness Plan Membership Agreement

360 Med

This Membership Agreement (“Agreement”) is entered into between the undersigned patient (“Patient” or “Member”) and 360 Med (“Clinic”), and outlines the terms and conditions of the Core Wellness Plan Membership. By subscribing to this plan, the Patient agrees to the following:

1. Membership Overview
The Clinic offers a premium monthly membership at the rate of £845 / Year. Members are entitled to the following benefits:

a) Annual Health Check Examination
A 30-minute face-to-face GP appointment to assess overall health, performed annually.

b) Comprehensive Blood Test
Included as part of the annual health check, this blood test covers a wide range of markers to evaluate health status.

c) 10 GP Appointments per Year
Members can access up to 10 GP consultations per year, either face-to-face or remote (telephone/video), for any general health concern.
Any required follow-up appointment after the health check or blood test will be taken from these included consultations.

d) Free Repeat Prescriptions
Repeat prescriptions provided at no additional charge, subject to medical need and GP approval.

e) Free Specialist Referrals
Where clinically indicated, specialist referrals will be issued free of charge.

f) 10% Discount on Additional GP Appointments and Investigations
This discount applies to:

Any GP consultations beyond the 10 included

Further investigations such as blood tests, scans, and diagnostics

g) 10% Discount on GP Appointments for Children Under 16
Members receive a 10% discount on GP appointments for their children (under age 16) for the duration of their membership.

2. Term and Termination
Minimum Term: 12-month commitment, beginning on the sign-up date.

Cancellation: May be cancelled after the initial 12-month term with 30 days’ written notice. Membership will continue until the end of the current billing cycle; no refunds will be issued for unused time.

3. Appointment Scheduling
a) Health Check & Blood Test
To be scheduled in advance, subject to availability, once annually.

b) GP Consultations

Members may book up to 10 GP appointments per year, subject to availability.
Consultations may be used for any health concern, including follow-ups.
Additional GP appointments may be booked with a 10% discount.

c) Further Investigations
Discounted at 10%, applicable to diagnostic tests such as blood tests, imaging, or specialist assessments.

4. General Terms
Eligibility: Available to individual patients aged 18 or over.
Non-Transferable: Membership benefits apply solely to the named Member and cannot be shared or transferred.
Children’s Discount: 10% discount applies to Member’s children under 16.
Non-Refundable: Membership fees are non-refundable and non-transferable.

5. Confidentiality and Privacy
All patient data will be handled confidentially in accordance with 360 Med’s Privacy Policy and relevant UK data protection legislation.

6 Liability
The Clinic is not liable for missed appointments or outcomes related to failure to follow medical advice. The Patient remains responsible for their health and engagement in care.

7. Changes to Terms
The Clinic may modify terms, fees, or benefits after the initial 12-month term.
All changes will be communicated in writing and take effect upon renewal.

8. Governing Law
This Agreement is governed by and shall be construed in accordance with the laws of the United Kingdom.

9. Acknowledgement
By checking the above box, the Patient confirms they have read, understood, and agreed to the terms and conditions of this Core Wellness Plan Membership Agreement.

Complete Family Package Membership Agreement

360 Med

This Membership Agreement (“Agreement”) is entered into between the undersigned patient (“Patient” or “Member”) and 360 Med (“Clinic”), and outlines the terms and conditions of the Complete Family Package Membership. By subscribing to this plan, the Patient agrees to the following:

1. Membership Overview
The Clinic offers a comprehensive family membership option at the rate of £1824 / Year, which provides coverage for up to 5 family members (2 adults and 3 children under the age of 18). As a Complete Family Package Member, the Patient and their family will receive the following benefits:

a) Annual Health Check for Adults
A 30-minute face-to-face GP appointment for each adult to assess overall health.

b) Comprehensive Blood Test for Adults
Included as part of the annual health check for each adult, covering a broad panel of tests.

c) 25 GP Appointments per Year
Members are entitled to 25 GP consultations per year, either face-to-face or remote (telephone/video), to be shared among the listed family members.

If follow-up appointments are required after the health check or blood test, these will be taken from the allocated 25 consultations.

d) Free Repeat Prescriptions
Available to all covered members, subject to medical necessity and GP approval.

e) Free Specialist Referrals
Provided as clinically indicated, at no additional cost.

f) 10% Discount on Additional Appointments and Investigations
Applies to:

Additional GP consultations beyond the included 25

Investigations such as blood tests, scans, and diagnostic procedures

g) Children Under 18
Children listed under the plan receive the same benefits, including access to shared GP appointments, repeat prescriptions, and specialist referrals.

2. Term and Termination
Minimum Term: 12-month minimum commitment from the date of sign-up.

Cancellation: After the 12-month minimum term, the Patient may cancel with 30 days’ written notice. Membership will continue until the end of the current billing cycle; no partial refunds will be issued.

3. Appointment Scheduling
a) Health Checks & Blood Tests (Adults Only)
Must be scheduled in advance and provided annually. Children’s GP appointments must also be booked ahead.

b) GP Consultations
Up to 25 appointments annually, shared between listed family members.
Appointments may be used for a variety of medical concerns.

c) Additional Services
Additional appointments are available with a 10% discount.
Further investigations (e.g., imaging, tests) also receive a 10% discount.

4. General Terms
Eligibility: For families including up to 2 adults and 3 children under 18. Benefits apply only to listed members.

Non-Transferable: Membership and its benefits cannot be transferred or shared with individuals outside of the listed family.

Non-Refundable: Membership fees are not refundable.

5. Confidentiality and Privacy
All personal health information is handled confidentially in accordance with 360 Med’s Privacy Policy and relevant data protection regulations.

6. Liability
The Clinic is not liable for missed appointments or consequences of not following medical advice. The Patient is responsible for coordinating the care of all family members under the plan.

7. Changes to Terms
The Clinic may amend this Agreement (e.g., pricing, services) after the initial 12-month term.

Any changes will be provided in writing before taking effect.

8. Governing Law
This Agreement is governed by the laws of the United Kingdom.

9. Acknowledgement
By checking the above box, the Patient confirms that they have read, understood, and agreed to the terms of the Complete Family Package Membership Agreement with 360 Med.

Concierge Care Membership Agreement

360 Med

This Membership Agreement (“Agreement”) is entered into between the undersigned patient (“Patient” or “Member”) and 360 Med (“Clinic”), and outlines the terms and conditions of the Concierge Care Membership. By subscribing to this service, the Patient agrees to the following terms:

1. Membership Overview
The Clinic offers a premium Concierge Care service at the rate of £1824 / Year. As a Member, the Patient is entitled to the following benefits:

Annual Health Check Examination with a GP
A 30-minute face-to-face appointment with a GP to assess overall health.

Comprehensive Blood Test
Included as part of the annual health check, covering a broad panel of tests.

10 GP Home Visits per Year
Within a 3-mile radius of the Clinic, these can be scheduled for various health concerns.

5 Telephone Consultations per Year
To discuss health concerns or seek advice with a GP.

Free Repeat Prescriptions
Subject to medical necessity and GP approval.

Free Specialist Referrals
Provided as clinically appropriate, at no extra cost.

10% Discount on Further Investigations
Includes blood tests, scans, and other diagnostics beyond those included in the plan.

2. Term and Termination
Minimum Term: A minimum 12-month commitment from the date of sign-up.

Cancellation: After 12 months, the Patient may cancel with 30 days’ written notice. The plan continues until the end of the billing cycle; no partial refunds are issued.

3. Appointment Scheduling
Health Check, Blood Test & Follow-Up
Must be scheduled in advance and are included annually. Follow-up appointments may be scheduled as needed.

GP Home Visits
Up to 10 visits per year within a 3-mile radius of the Clinic, scheduled in advance and subject to availability.

Telephone Consultations
Up to 5 per year, must be pre-booked.

Additional Appointments
Additional GP consultations and investigations receive a 10% discount.

4. General Terms
Eligibility: This membership is for individual patients only.

Exclusivity: Benefits are non-transferable and apply solely to the listed Member.

Non-Refundable: Fees are non-refundable.

5. Confidentiality and Privacy
The Clinic handles all patient information with strict confidentiality, in accordance with the Clinic’s Privacy Policy and UK data protection laws.

6. Liability
The Clinic is not responsible for missed appointments or consequences arising from the Patient not acting on medical advice. The Member is responsible for managing their health appropriately.

7. Changes to Terms
The Clinic may modify this Agreement after the initial 12-month period.

All changes will be communicated in writing in advance.

8. Governing Law
This Agreement is governed by the laws of the United Kingdom.

9. Acknowledgement
By checking the above box, the Patient confirms that they have read, understood, and agreed to the terms of the Concierge Care Membership Agreement.
